When you call to schedule an appointment, please provide us with a claim number & insurance company phone number. If you have an attorney assisting you in your claim, please provide us with their name and number as well.
Work Related Injuries (State and Federal Work Comp):
For patients that have been injured while on the job, there are certain requirements for seeking treatment. First, you need to notify your employer as soon as possible that you've been injured. Second, simply ask the employer if you can seek your own physician. If they agree, contact us to schedule a consultation. If you are already seeing a physician and would like would like to see Dr. Snider instead, it may be possible to make a change. If you are a federal employee and have been injured on the job, we also accept Department of Labor/Federal Worker's Comp Claims in most cases.
