We accept all PPO and non-HMO plans on an "Out of Network" basis. This means that you do not have to have a referral to be seen in our office. When there is an Out of Network deductible, we will accept 1/2 of the deductible amount and will "write off" the other 1/2 as a courtesy.
Self Pay:
For patients that do not have health insurance, or that have extremely high deductibles, we offer the first visit for $200.00 and $50 for subsequent visits. We can also offer a discounted "inclusive" treatment plan for patients that would like to pre-pay for their services. The amount would be determined based upon the individual treatment plan that is recommended following the initial evaluation.
